Education Market Combo Deck Rebate

To receive reimbursement from JVC, the customer must be a bona fide educational institution.

To qualify for a rebate, you must be a bona fide educational institution in the U.S. or Puerto
Rico, and purchase a JVC SR-MV40US, SR-MV50US, SR-DVM600US or SR-DVM700US
combination deck, new, from an authorized JVC PROFESSIONAL PRODUCTS COMPANY
DEALER between November 1, 2006 and March 30, 2007.

The completed claim form must be received by JVC no later than April 30, 2007.

Please allow 6 to 8 weeks for processing of the rebate from the time the claim has been received by
JVC Professional Products Company. JVC reserves the right to determine if you are a qualified
educational institution and may ask for further documentation.
